Bronchiolitis obliterans syndrome (BOS) is a major complication of lung transplantation that is associated with poor survival. The International Society for Heart and Lung Transplantation, American Thoracic Society, and European Respiratory Society convened a committee of international experts to describe and/or provide recommendations for 1) the definition of BOS, 2) the risk factors for developing BOS, 3) the diagnosis of BOS, and 4) the management and prevention of BOS.

A comprehensive review of the literature on lung transplantation and BOS allowed the committee to reach a number of conclusions. In addition, committee members used a systematic approach to formulate a number of specific evidence-based recommendations for the prevention and management of BO. Evidence tables are provided in the online supplementary material. It is our hope that this guideline will promote an understanding of the current approach to the evaluation and management of lung transplant recipients who develop delayed allograft dysfunction, as well as stimulate additional research that will provide higher quality evidence upon which future guidelines may be based.
