
Become a Fellow of the ISHLT
Submit an application by 15 December
The Fellow of the ISHLT (ISHLTF) Program is an honorary post-nominal designation recognizing dedicated ISHLT members and their contributions to the field as well as their commitment and service to the Society. The first group of ISHLTFs will be inducted at the ISHLT Annual Meeting & Scientific Sessions in Toronto in April 2026.
Applications for ISHLTF are being accepted through Monday, 15 December. Self-nominate using the link below.
NOTE: Applications will be accepted until 11:59 ET on Monday, 15 December / 04:59 UTC on Tuesday, 15 December. ISHLT recommends that applicants complete the application in one sitting.
Eligibility Criteria
- Must be an ISHLT member ≥ 5 years, actively engaged in the field.
- Must accumulate a minimum number of points within a four-part point qualification system:
- Core Qualifications
- Member Engagement
- Service to ISHLT
- Commitment to the Field
The point-based system is designed to provide clarity, flexibility, and accessibility for ISHLT members across different professional backgrounds and geographic regions.
- Applicants must submit:
- Two letters of recommendation
- A CV, resume and/or publications bibliography
- A one-time non-refundable application fee (based on economy and reduced for non-physicians). NOTE: The fee will be used to support the ISHLT Foundation.
Why Become an ISHLTF?
ISHLTF allows ISHLT members to be recognized among their peers and colleagues for their exemplary dedication to the field of heart and lung disease, transplantation, and for their service to ISHLT.
ISHLTFs:
- Will be honored before their peers during a General Session at the first ISHLT Annual Meeting after their acceptance
- May use the ISHLTF designation after their name
- Receive a certificate and ISHLTF pin to recognize their credential
- Will be denoted as ISHLTF on ISHLTconnect and in future ISHLT programming
- Will be included in a list of ISHLTFs on the ISHLT website

- 15 February, 2026: Notification of results
- April 2026: Fellows inducted at the ISHLT Annual Meeting
Once submitted, ISHLT staff confirm that the application is complete. The ISHLT Membership Committee reviews the letters of recommendation and makes final determinations on awarding the designation of Fellow of ISHLT (ISHLTF). Your application will be marked as “pending” in your ISHLT member profile until you are notified of your status in February.
Although you may revisit your application by logging into your member account member account, ISHLT cannot ensure that all of your application details will be saved. Therefore, ISHLT recommends that candidates complete the application in one sitting.. You may access the full application criteria before visiting the online application.
Two letters must be submitted:
- One from either an ISHLT leader (elected or appointed) or a mentor/department supervisor.
- One from any ISHLT member in good standing.
Letters of recommendation cannot be written by current members of the ISHLT Membership Committee, ISHLT Board of Directors, or the ISHLT Foundation Board.
We recommend you upload your letters with the following naming convention: “Last Name_First Name_Document Type”
Letters of recommendation should detail accomplishments relevant to the ISHLT criteria, impact of your contributions to the organization and/or profession, and rationale for why you should receive the ISHLTF.
Only the following can be submitted:
- Two letters of recommendation
- A CV, Resume, and/or publications bibliography
If, for example, you have a Resume and Publications Bibliography, they will need to be combined in the same document and uploaded into the system.
