Register for the ISHLT Virtual On Demand 2022 Annual Meeting and Scientific Sessions

Now more than ever, the ISHLT community—multidisciplinary, global, and diverse—really matters. For anyone who was unable to participate at the in-person Annual Meeting in Boston, you can still see the latest science and stay current on research with our Virtual On Demand Registration.

Accessing the Virtual Meeting
Registration for the In-Person Meeting in Boston includes access to all of the on demand meeting content. Registrants of the In-Person and Virtual Meeting will be able to begin viewing meeting content on-demand on 31 May, 2022 and will have access to the virtual platform for six months to give you the maximum time to access the sessions and watch the content on your schedule. You can also message with attendees and presenters using the discussion board in each session.

Earn CME/CE Credits for On Demand Content
With Virtual On Demand Registration, attendees will also have the ability to earn continuing education credits, just like you would at a live meeting! If you were at the meeting in Boston, access to the on-demand content and the opportunity to claim additional CME/CE credit is included in your In-Person Registration fees. More details about the exact number of available credits and how to earn them will be available on 31 May, 2022.

Registration Rates and Instructions

  •   Industry Rate is available to anyone whose primary employer is a for-profit commercial entity, excluding medical centers and healthcare providers. Individuals who meet this definition may not register under any other category.
  •   Physician/Surgeon Rate is available to individuals who hold an MD degree or equivalent.
  •   Allied Health/Non-Physician Rate is available to individuals who do not hold an MD degree or the equivalent (e.g., Non-MDs, PhDs, researchers, nurses, pharmacists, physical therapists, psychologist, social workers, etc.) and are not pharmaceutical or device company employees or consultants.
  •   Non-member Allied Health Registrants must include a letter verifying employment and allied health status with their registration. Registration submitted without this letter will not be processed. Email the verification letter to registration@ishlt.org.
  •   Student/Trainee Rate is available only to individuals who are actively participating in a formal training program (e.g., medical, graduate and nursing students, residents, fellows, or the equivalent).
  •   Non-member Student/Trainee registrants must include with their registration a letter from their institution documenting their status as an individual in training. Registration submitted without a letter will not be processed. Email the verification letter to registration@ishlt.org.

Registration Policies and Instructions

PAYMENT OF REGISTRATION FEES
  •   Registration fees are determined by the date when payment in full is received. Registration submitted without payment in full, including declined credit card transactions, are subject to the registration fee in effect at the time payment is received in full.
  •   Full payment in U.S. funds only must accompany your registration. Checks must be made payable to ISHLT and drawn on a U.S. bank. Bank fees incurred for processing payment will be billed to the registrant.
  •   Wire transfers must be received by 8 April, 2022. A $35 USD fee will be accessed for each wire transfer. These fees are the responsibility of the sender. Email registration@ishlt.org for wire transfer instructions.
  •   Registrant substitutions must be received in writing by 8 April, 2022. Only one complimentary substitution will be permitted until 8 April; a $25 USD fee will be assessed for each additional substitution. If the substitution isn’t of the same registration category, the difference in the registration rate must be paid or ISHLT will refund the difference. After 8 April, a $25 USD fee will be assessed for each substitution. Email registration@ishlt.org for registrant substitutions.
     
REGISTRATION CANCELLATION Registration cancellations must be made in writing by submitting an email to registration@ishlt.org, noting the specific reasons for the registration cancellation.

REFUNDS
All refunds will be issued 30 days after the conclusion of the meeting.

INVOICE REQUESTS
Registration will be subject to a $25 USD administrative fee when requesting an invoice to pay for registration. Payments must be received or postmarked by 8 April. ISHLT does not accept purchase orders.

PAYMENT CONFIRMATION
Attendees who include an email address on their registration form will receive an email confirmation and receipt within 24 hours of registering online.

PRIVACY POLICY
Your data privacy and security are important to us. When you register for this event, ISHLT will process your data in accordance with our legitimate interests for hosting an industry event and for being a community-based organization. Please review our privacy policy, including how to exercise your rights under the GDPR.

LIABILITY WAIVER + BE WELL AGREEMENT
As a condition of my participation in this meeting or event, I hereby waive any claim I may have against the ISHLT and its officers, directors, employees, or agents, or against the presenters or speakers, for reliance on any information presented and release ISHLT from and against any and all liability for damage or injury that may arise from my participation or attendance at the program. I further understand and agree that all property rights in the material presented, including common law copyright, are expressly reserved to the presenter or speaker or to ISHLT. I acknowledge that participation in ISHLT events and activities brings some risk and I do hereby assume responsibility for my own well-being. If another individual participates in my place per ISHLT transfer policy, the new registrant agrees to this disclaimer and waiver by default of transfer.

I understand that travel and gathering involves risk of sickness, including sickness from COVID-19. I (and on behalf of my guest(s)) waive and release ISHLT and its sponsors and exhibitors, and their employees and agents, from and against claims, liabilities and expenses arising from injury, sickness or death from contraction or spread of COVID-19 or other communicable disease due to travel to or attendance at an event sponsored by ISHLT.

I will take necessary precautions while at the event including, but not limited to, engaging in appropriate social distancing, wearing a mask when requested and/or required, minimize face touching, frequently washing hands and avoiding risky environments such as overcrowded bars or restaurants. I agree to not attend any ISHLT event if I feel ill or had recent exposure to a COVID-19 case.

This waiver and release is binding on me and my heirs and successors.

CODE OF CONDUCT
ISHLT is a listening organization focused on its participants. ISHLT2022 is designed to increase interaction, engagement, collaboration, connectivity, and community, in a fun and safe learning environment.

We value the participation of each member of the community and endeavor to deliver an enjoyable and fulfilling experience. Conference participants are expected to conduct themselves with integrity, courtesy and respect for others and maintain the highest level of professionalism at all conference programs and events, whether officially sponsored by ISHLT or not.

All attendees, guests, speakers, organizers, volunteers, partners, vendors, and staff at any ISHLT event are required to observe the following Code of Conduct. ISHLT is dedicated to providing a harassment-free conference experience for everyone, regardless of age, gender, gender expression, sexual orientation, disability, physical appearance, body size, place of origin, race, or religion. We do not tolerate harassment of conference participants in any form.

All communication should be appropriate for a professional audience including people of diverse backgrounds and cultures. Sexual language and imagery is not appropriate for the conference.

Be kind to others. Do not insult or defame participants. Harassment in any form, sexist, racist, or exclusionary jokes are not condoned at ISHLT Events. Participants violating these rules may be asked to leave the conference at the sole discretion of ISHLT. Thank you for helping to make this a welcoming event for all.

DISCLAIMER AND WAIVER
ISHLT intends to take photographs and video of this event for use in ISHLT news and promotional material, in print, electronic and other media, including the ISHLT website. By participating in this event, I grant ISHLT the right to use any image, photograph, voice, or likeness, without limitation, in its promotional materials and publicity efforts without compensation. All media become the property of ISHLT. Media may be displayed, distributed, or used by ISHLT for any purpose.

During the registration process, you will be given the option to opt-in to have your information shared for the following reasons:
 
  •   Attendee Listing: ISHLT would like to publicly list you as an attendee. This helps drive attendance, and we kindly ask for your participation. If you agree, we will list your name, title, company, and location, on the website and in the attendance area of the mobile app and virtual platform.
  •   Additional ISHLT Information: ISHLT would like to share information regarding membership and programs, unrelated to this meeting.
  •   Sponsor Information: With the support of our sponsors, ISHLT2022 is taking place in Boston. I agree that my information may be shared so that ISHLT2022 sponsors can contact me to promote their business and services.
